Any requests for disclosure or correction of customers' personal data held by BofA Securities Japan Co., Ltd. (the Company) following the Personal Information Protection Law shall be in accordance with such procedures as the Company designates including a confirmation of identification of the "Principal" of the said personal data, and completion of a request form described below.

Please fill out all items in the "Personal Information Disclosure and Correction Request Form," seal or sign it, and submit it to Compliance.
For customers who do not have an account with the Company
Identification of the Principal will be confirmed from the signature or the registered seal attached on the "Personal Information Disclosure and Correction Request Form."
For customers who do not have an account with the Company
Documents to confirm identification of the Principal shall be submitted to the Company together with the "Personal Information Disclosure and Correction Request Form."
For each request for disclosure or correction, a standard fee established by the Company will be charged. In addition to such fees, actual cost may be harged if it exceeds the fees above, depending on the details of information required.
For customers who have accounts with the Company, such fees will be withdrawn against the balance of the customer's account. For customers who do not have an account with the Company, payment of such fees shall be transferred to a bank account designated by the Company.
Relevant items for disclosure or correction of personal data, such as reason for request, period, name of department or office handling the said personal data, term, etc., shall be entered in full in the "Personal Information Disclosure and Correction Request Form."
Upon receiving the fully completed "Personal Information Disclosure and Correction Request Form" and payment of the requested fees, the Company will promptly send a reply by mail to the customer's forwarding address.
